Effect of a Home-based Versus Supervised Exercise Program in Patients With Migraine
Sponsor: University of Gran Rosario
Summary
Migraine is a common primary headache with documented impacts on the patient, the economy, and society. A growing number of studies have reported that people with migraines are more likely to experience neck pain and neck dysfunction. The coexistence of migraine and neck pain is also associated with a higher frequency of migraine attacks, greater susceptibility to certification. It has been reported that they present decreased cervical mobility, as well as decreased strength, resistance, and coordination of the cervical musculature of the skull. The objective of this study is to evaluate the possible clinical effects in migraineurs of supervised craniocervical and axioscapular exercises versus home-based exercises.

Interventions
Experimental
Stage 1 (weeks 1-4): Craniocervical flexion exercise (CCFE; 3×10, 10-s holds, progressing from lower to higher complexity); isometric cervical flexion and extension (IFE/IEE; 3×8, 6-s holds); and resisted cervical extension with an elastic band (REE), flexion against a wall (RFE), and rotation using manual resistance (RRE; 3×8, 6-s holds). Isometric exercises are performed with the head and spine in neutral. Stage 2 (weeks 5-8): The same exercises plus low-row and mid-row with an elastic band (3×8 each). Sessions end with static stretching of cervical and scapulothoracic muscles (3×8, 30-s holds). Resistance is individually adjusted by the physiotherapist.
Control
Home-based exercise: Participants perform the same 8-week programme as the supervised group, twice weekly (60 min/session), without direct supervision. At the initial session, a physiotherapist explains exercise technique and progression and provides printed materials and instructional videos. Stage 1 (weeks 1-4): CCFE (3×10, 10-s holds) and IFE/IEE, REE, RFE and RRE (3×8, 6-s holds, neutral head and spine). Stage 2 (weeks 5-8): the same exercises plus elastic-band low-row and mid-row (3×8 each). Sessions end with static stretching of cervical and scapulothoracic muscles (3×8, 30-s holds). Adherence is monitored weekly using an online form to report completion of the two scheduled sessions.
