Guiding Optimal Blood Pressure Management in Peritoneal Dialysis Pilot Trial
The GOAL-BP pilot study will help determine whether a study comparing two blood pressure goals can be carried out in adults with end-stage kidney disease and high blood pressure who receive peritoneal dialysis. The main questions it aims to answer are whether eligible patients can be recruited for the study. Researchers will compare two goals for the top blood pressure number (systolic blood pressure): below 120 mmHg versus below 140 mmHg.This pilot study will help plan a larger trial in the future. Participants will: * Be assigned by chance to one of the two blood pressure goals. * Have their blood pressure measured at clinic visits about every two weeks. * Work with their kidney doctor on recommended changes to fluid management, peritoneal dialysis treatment, or blood pressure medicines to reach their assigned goal. * Be part of the study for about four and a half months.

Under Pressure: Arugula Extract's Effect on Blood Pressure in Hypertensive Individuals
The purpose of this randomized, single-blind, placebo-controlled, crossover study was to investigate the acute effects of arugula extract (AE) supplementation on blood pressure, endothelial function, and circulating nitric oxide (NO) metabolites in individuals with elevated to stage 1 hypertension. Hypertension is a major contributing factor to cardiovascular disease (CVD) and is commonly associated with endothelial dysfunction and impaired NO bioavailability. Dietary nitrate supplementation has demonstrated beneficial effects on cardiovascular health through the nitrate-nitrite-NO pathway. Although nitrate-rich supplements such as beetroot juice have been extensively investigated, limited research has examined the vascular effects of arugula extract in individuals with hypertension. Sixteen adults between the ages of 18 and 50 years with elevated to stage 1 hypertension participated in the study. Participants completed two experimental conditions in randomized order: 4,000 mg of arugula extract, providing approximately 400 mg of dietary nitrate, and 4,000 mg of psyllium husk placebo. Each condition was separated by a minimum 72-hour washout period to minimize potential carryover effects. During each experimental condition, participants underwent baseline cardiovascular and vascular assessments before consuming the assigned supplement. Systolic blood pressure, diastolic blood pressure, heart rate, and venous blood samples were collected at baseline and 1, 2, 3, and 24 hours following supplementation. Endothelial function was assessed through brachial artery flow-mediated dilation (FMD) at baseline, 3 hours, and 24 hours post-supplementation. Blood samples were analyzed to assess circulating nitrate, nitrite, and total NO metabolites. The primary objective was to determine whether acute arugula extract supplementation improved endothelial function and reduced blood pressure compared with placebo. Secondary objectives included evaluating changes in circulating NO metabolites. This investigation sought to determine the potential of arugula extract as a dietary nitrate intervention for improving vascular health in individuals with elevated blood pressure.

Stopping Blood Pressure Medications Without Tapering in Long-Term Care
Blood pressure medications may provide limited benefit and cause side effects in frail older adults. It is unclear how best to stop these medications. Guidelines often recommend gradually reducing the dose, but this process can be time-consuming. This study examined the feasibility of stopping blood pressure medications without gradually reducing the dose in long-term care residents with low blood pressure. Eligible residents had their medication stopped and were monitored for four weeks.

Evaluation of a New Virtual Care Pathway for Older Adults With Noncommunicable Diseases
The purpose of this study is to evaluate a newly developed virtual care program for older adults with chronic diseases (hypertension, diabetes and lipid disorders) whether it is feasible and economical to implement in the Malaysian primary healthcare settings. Investigators hypothesize that the use of this virtual care program (TeleCOOP-NCD©) will be more cost-effective (lower total costs per benefits gained) compared to the usual onsite care at the clinic for older adults who have stable chronic diseases. The investigators also hypothesize that there is no significant difference in control of clinical parameters compared to usual care. Researchers will compare the primary outcomes such as patient satisfaction, health-related quality of life, provider cost and patient cost between the intervention and control group. Researchers will also check participant recruitment rate, participant retention rate and clinical parameters (including blood pressure, hemoglobin A1c, fasting blood glucose, fasting serum lipids) as secondary outcomes. Eligible participants will receive either virtual care program (intervention) or usual care (regular in-person visits) at a primary care clinic, called the Klinik Primer of Hospital Canselor Tuanku Muhriz (KP-HCTM), in Bandar Seri Permaisuri, Kuala Lumpur, Malaysia. Researchers will do outcome assessments at baseline, 3 months and 6 months respectively.

Mortality Outcome of Controlled Blood Pressure Participants
Despite advances in antihypertensive therapy, some patients with well-controlled blood pressure still experience fatal outcomes. This paradox raises critical questions about the efficacy of blood pressure-targeted interventions and whether BP control alone ensures survival. This systematic review and meta-analysis aims to evaluate all-cause mortality and cardiovascular death among hypertensive patients who achieved BP targets in RCTs or open-label trials. To determine whether achieving controlled blood pressure through intentional intervention (pharmacological or procedural) in RCTs or open-label studies reduces mortality (all-cause and cardiovascular) in hypertensive patients.

Spinal Cord Stimulation for Blood Pressure Control in Hypertension
High blood pressure, also called hypertension, raises the risk of heart disease, stroke, and kidney disease. Medications work well for many people, but some people still have blood pressure that stays too high, or they have side effects from their medications. Researchers are looking for treatments that do not involve taking more medication. Nerves help control blood pressure by changing how blood vessels tighten and how hard the heart pumps. Because of this, treatments that act on nerves may be able to lower blood pressure. Spinal cord stimulation is a treatment already used to manage long-term pain. It uses a small device, placed under the skin during a standard operation, that sends mild electrical signals to the spine. Laboratory studies suggest that stimulating the lower part of the mid-back may also lower blood pressure, but this has not been carefully studied in people who already use these devices. This study will look at whether changing the settings on a spinal cord stimulator affects blood pressure. About 11 adults who have high blood pressure and who already have a spinal cord stimulator for long-term pain. No new device will be placed, and no surgery or other invasive procedure will be done for this study. Participants attend a series of study visits. At each visit, trained staff record blood pressure and heart rate while the stimulator is turned on, turned off, and set to other settings that the person's care team has approved and that the person finds comfortable. There are rest breaks between each setting. Over successive visits, the study team works to identify the settings that best lower blood pressure. Participants can stop at any time, and their usual settings are restored at the end of each visit. The study will also look at whether this kind of testing is practical and comfortable for participants. The results will help researchers decide whether larger studies of spinal cord stimulation for blood pressure control are worth doing.

The BASIC-S Feasibility Study for Vascular Risk Reduction
Vascular risk factors drive stroke, heart disease, and dementia, but translating lifestyle guidelines into routine clinical care remains challenging. To address this, we developed BASIC-S (Blood Pressure, Activity, Sleep, Interaction, Consumption, and Support), a framework targeting Blood pressure, Activity, Sleep, Interaction (socially), Consumption (nutrition), and Support using partner support and motivational interviewing. This individual-focused approach bypasses the logistical and financial barriers of complex clinical interventions by using sustained behavioral science to support gradual lifestyle changes. This study is a single-center, mixed-methods, cluster-randomized pilot trial evaluating the feasibility of the BASIC-S protocol in an occupational setting. A maximum of 60 households (120 participants) will be randomized 1:1 to either the BASIC-S intervention (motivational coaching, secure video check-ins, automated text messages, and a digital workbook) or standard care. Outcomes will assess feasibility, acceptability, and the validity of new tracking metrics to inform a future full-scale trial.
