A Novel Treatment for Chronic Low Back Pain Using Multi Angular Isometric Lumbar Exercise: Randomized Clinical Trial.

The study aims to evaluate the effectiveness of Multi Angular Isometric Lumbar Exercise (MAILE) in comparison to traditional lumbar stabilization and core stability treatments for managing nonspecific chronic low back pain (NCLBP). MAILE is designed to specifically target the Lumbo-Pelvic Hip complex with the goal of improving pain levels, stability, strength, and overall function. Objectives of the study are to assess several key outcomes: Pain Intensity: Measuring the level of pain experienced by patients. Functional Disability: Evaluating the impact of NCLBP on daily activities and overall function. Functional Stability: Assessing the stability of the Lumbo-Pelvic Hip complex during movement. Muscle Strength: Determining the strength of muscles within the targeted region. By examining these outcomes, the research seeks to determine whether MAILE offers superior benefits and greater cost-efficiency compared to conventional treatments for patients with NCLBP and motor control dysfunction. If successful, MAILE could provide a more effective and economical approach to managing this common and debilitating condition. This Randomized Clinical Trial (RCT) will be conducted across multiple outpatient clinics within the Hamad Medical Corporation (HMC). The study will involve 98 patients diagnosed with non-specific chronic low back pain (NCLBP), who will be randomly assigned to one of two groups: the Multi Angular Isometric Lumbar Exercises (MAILE) group or the Core Stability group. Both groups will follow tailored exercise programs over a three-week period. The MAILE group will focus on exercises designed to enhance lumbar stability and core strength through multi-angular isometric lumbar exercises performed at various angles. These exercises aim to target the deep stabilizing muscles of the lumbar spine, promoting overall spinal stability and reducing pain. The Core Stability group will engage in exercises that emphasize the activation and strengthening of the core muscles, which play a crucial role in maintaining spinal stability and function. The outcomes of this trial will help determine the effectiveness of MAILE compared to traditional core stability exercises in managing NCLBP, potentially guiding future therapeutic strategies.

Multimodal Imaging and Biospecimen Collection for Low Back Pain (LBPB)

This prospective cohort study investigates the neurobiological, genetic, and psychosocial mechanisms underlying acute and chronic low back pain (LBP). Core objectives include establishing a high-quality biobank to support future research in connectomics, genomics, and biomarker discovery, and identifying predictors of pain progression and treatment response. The study will also assess the impact of comorbid conditions such as anxiety, depression, and sleep disturbances on pain perception and clinical outcomes. Longitudinal analyses will explore the dynamic interplay between emotion, cognition, sleep, and pain to inform precision, mechanism-based interventions. Functional imaging will be used to examine brain responses to nociceptive modulation, aiming to identify neural circuits involved in pain chronification. By integrating multimodal data-including neuroimaging, neurophysiology, microbiota profiling, polysomnography, and molecular assays-the study will define LBP subtypes, with a particular focus on nociceptive, neuropathic, and nociplastic mechanisms. The ultimate goal is to establish prognostic biomarkers and advance personalized strategies for LBP prevention and treatment.

Muscle Energy Technique in Patients With Nonspecific Low Back Pain

The aim of this Randomized control trial is to find out the comparative effects of muscle energy technique on iliotibial band versus hamstring and abdominal muscle activation in patients with non specific low back pain in improving pain intensity and functional disability. The sample size was 28. Subjects are randomly divided into two groups. 14 subjects in muscle energy technique group and 14 in hamstring and abdominal muscle activation group. Study duration was of 6 months. Sampling technique applied was Non probability purposive sampling. Subjects between the age group 18- 40 years with acute non specific low back were included. Tools used are Numeric pain rating scale and Oswestry disability index.
