Impact of a New Bioceramic Based Intracanal Medication

Periapical lesions are the commonly occurring pathologic lesions that affect the periradicular tissues of the tooth. This lesion can be classified as granuloma, cystic, or abscess .The pathological changes seen in some periradicular and periapical cases are due to the presence of microorganisms, and their byproducts. These released toxins have the ability to diffuse through inflamed pulpal tissue and get into the periapical area. In a necrotic pulp, the lack of blood flow in the root prevents host immunity from eliminating the infection.

Comparison of Music and Ambient Noise Cancellation in Patients Undergoing Total Knee Arthroplasty (TKA)

The aim of this clinical trial is to compare the effects of music and ambient noise cancellation during total knee arthroplasty and to analyse the effects on patients. The study will also collect information on patients' functional knee scores and anxiety levels. The main questions to be answered are the following: Does the blocking of music or ambient noise, which is predicted to reduce anxiety, reduce people's anxiety levels? To what extent are participants affected by ambient noise? Does music or ambient noise blocking lead to an improvement in patients' functional scores? The researchers will work with 3 groups of patients who will be exposed to ambient noise blocking, music playing and ambient noise during knee replacement surgery. Participants will do the following: Be asked questions about anxiety and knee function scores before and after surgery. Attend clinical examinations at specified times for checks and tests. Outcomes will be assessed and recorded at appropriate times.

The IMPACT Study: Personalized Physical Therapy for Better Recovery and a Stronger Core After Hernia Surgery

Ventral hernia repair is among the most common operations performed in the United States each year, yet the great majority of patients receive no structured rehabilitation for the persistent pain, weakness, and functional loss that can follow surgery. This project will establish whether a tailored, mind-and-body physical therapy program can be feasibly and equitably delivered within a novel trial design embedded in routine surgical care, generating the operational evidence needed to design a definitive efficacy trial. The results will directly inform post-operative care pathways, clinical practice guidelines, and payer coverage decisions for a surgical population exceeding 600,000 patients annually.

Perioperative Mirogabalin for Pain Relief After Modified Radical Mastectomy

This study aims to evaluate whether perioperative mirogabalin can reduce postoperative pain and morphine consumption in female patients undergoing modified radical mastectomy for breast cancer. Postoperative pain is common after breast cancer surgery and may increase the need for opioid medications such as morphine. Although mirogabalin has been used to treat certain types of nerve pain, evidence regarding its effectiveness for acute postoperative pain remains limited. This prospective, randomized, double-blind, placebo-controlled study will include 50 female patients aged 18-65 years with American Society of Anesthesiologists (ASA) physical status II or III who are scheduled for elective modified radical mastectomy at the National Cancer Institute, Cairo University. Participants will be randomly allocated to one of two groups. The mirogabalin group will receive 15 mg orally approximately 2 hours before surgery and every 12 hours for 48 hours after surgery. The control group will receive matching placebo capsules on the same schedule. Both groups will receive standardized general anesthesia and multimodal postoperative analgesia, including paracetamol and ketorolac, when appropriate. Intravenous morphine will be administered as rescue analgesia according to a predefined protocol when the postoperative pain score reaches 4 or higher on a 0-10 visual analogue scale. The primary outcome will be total intravenous morphine consumption during the first 24 hours after surgery. Secondary outcomes will include postoperative pain scores at rest and during movement, time to first rescue analgesia, the number of patients requiring morphine, postoperative nausea and vomiting, sedation, dizziness, respiratory safety, hemodynamic changes, recovery from anesthesia, patient satisfaction, and quality of recovery. The study will assess whether adding mirogabalin to standard multimodal analgesia improves postoperative pain control and reduces opioid requirements without unacceptable adverse effects. The findings may help clarify the potential role of mirogabalin in perioperative pain management for patients undergoing breast cancer surgery.

Cryoneurolysis Prior to Total Knee Arthroplasty

Knee osteoarthritis is a major issue worldwide with limited treatment options. Many patients receive knee joint replacement surgery which is considered effective and safe. Nonetheless the period post-surgery is characterized by moderate to severe pain inhibiting early ambulation, motivation and range of motion, compromising rehabilitation, patient satisfaction, and overall outcomes. An optimal strategy of postoperative pain treatment after knee replacement surgery has not yet been established. Recently, our research center and others have shown that is possible to target the nerves surrounding the knee with a novel treatment called cryoneurolysis. Cryoneurolysis apply low temperatures \[-20°C ; -100°C\] to a target nerve, which disrupts nerve function and provides potential pain relief. This suggest a potential for cryoneurolysis to significantly improve rehabilitation, reduce opioid intake and overall outcomes after knee replacement surgery. The primary objective of the current project is to determine the effectiveness of cryoneurolysis in its proposed ability to reduce opioid intake and postoperative pain after knee replacement surgery. The study is a randomized controlled study with two groups. Group CRYO receives cryoneurolysis prior to knee replacement surgery and group SHAM receives a sham treatment prior to knee replacement surgery. Both groups receive surgery, analgesics and postoperative rehabilitation as per usual. Efficacy of treatment is evaluated using the change in opioid intake in the CRYO group compared to the SHAM group 14 days after knee replacement surgery. Participants will also be assessed at 90 and 180 days after knee replacement surgery and will include measures on pain, quality of life and function.

Liposomal Bupivacaine and Transoral Robotic Surgery

This study is for patients with oropharyngeal squamous cell carcinoma. We want to learn more about how we can optimize pain control in patients who undergo transoral robotic surgery (TORS) for oropharyngeal squamous cell carcinoma. Our goal is to determine if a local anesthetic called EXPAREL® (Liposomal Bupivacaine) impacts postsurgical pain and swallow function in patients with oropharyngeal squamous cell carcinoma undergoing TORS. EXPAREL® is an FDA-approved anesthetic drug that provides long-lasting and precise pain relief when injected into the surgical wound. Our study team wants to determine if injecting EXPAREL® into the surgical wound will provide better pain relief and swallow function when compared to patients who do not undergo postoperative EXPAREL® injection. Both options for postoperative pain control are considered standard of care for patients undergoing TORS.

Postoperative Analgesia in Patients Undergoing Open Upper Abdominal Surgeries: External Oblique Intercostal Plane Block vs. Quadratus Lumborum Block

Open upper abdominal surgeries with subcostal incisions are a cause of severe pain and can lead to pulmonary and cardiac complications, detrimental physiological effects, and may also have psychological, economic, and social adverse effects if inadequately treated. Effective pain control can avoid these complications and contribute to several clinically valuable outcomes, including earlier patient mobilization and quicker recovery, which can result in a shortened hospital stay and reduced costs. Opioids are the gold standard in postoperative pain control. however, it increases the incidence of opioid related adverse events such as respiratory depression, dizziness, nausea, vomiting and constipation. Regional analgesia plays an important role in perioperative multimodal analgesic regimens for major abdominal surgeries. The ultrasound-guided technique provides several options for relieving postoperative pain. The aim of this study is to compare the efficacy and safety of ultrasound guided external oblique intercostal plane block versus quadratus lumborum in patients undergoing open upper abdominal surgeries.

Volume Dependent Effect of Pericapsular Nerve Block

In PENG block technique, the local anesthetic agent is injected between the psoas tendon and the pubic ramus to block the sensory branches of the nerves which innervates the hip capsule for providing analgesia without causing muscle weakness. Since the PENG block is a facial plane block, it causes a volume dependent distribution. There are a few case reposts and cadaveric studies on volume dependent effect of analgesia and motor weakness of PENG block. In our study, we aimed to evaluate the clinical effects of the volume-dependent efficacy of PENG block.

Erector Spinae Plane Block Versus Rectus Sheath Block for Postoperative Analgesia After Supraumbilical Surgery

Objectives: To compare the effectiveness of erector spinae plane block (ESPB) and rectus sheath block (RSB) in providing postoperative analgesia after supraumbilical surgery in adult patients and their impact on the patient's outcomes. Background: Supraumbilical surgery for hernia repair is the second-most-popular after surgical inguinal hernia repair and is accompanied by moderate to severe postoperative pain, so patients always require large doses of opioids within the first postoperative day. Because opioids have several adverse effects such as drowsiness, pruritus, nausea, and vomiting, regional analgesic techniques are an essential component of postoperative opioid-sparing analgesia. Previous studies have shown that regional analgesic techniques after abdominal wall surgeries can be an essential element of a postoperative pain management strategy with minimal adverse effects and hemodynamic responses. ESPB provides both somatic and visceral analgesia to the abdominal wall, through the blockade of the anterior rami of spinal nerves and the rami communicants involving sympathetic nerve fibers. RSB provides analgesia to the anterior abdominal wall from the xiphoid process to the symphysis pubis, through the blockade of the anterior rami of the 7th to 12th intercostal nerves. The dermatomal distribution of ESPB and RSB makes them ideal regional analgesic techniques after abdominal surgery, and to our knowledge, there were no previous trials that studied the difference between them. Patients and Methods: This was a prospective, randomized (1:1), double-blind clinical trial on 60 patients scheduled for elective supraumbilical surgery under general anesthesia at our hospital. Patients will be randomly allocated into two equal groups (30 patients each) and will receive: in group E; general anesthesia with postoperative bilateral ultrasound-guided ESPB, whereas in group R; general anesthesia with postoperative bilateral ultrasound-guided RSB.

Insulin Eye Drops and Epithelial Healing After Corneal Cross-linking

The goal of this clinical trial was to learn if topical insulin eye drops affected corneal epithelial wound healing after corneal collagen cross-linking. The main questions it aimed to answer were: Does topical insulin accelerate corneal epithelial healing after corneal collagen cross-linking? Does it affect post-operative pain? Participants were all diagnosed as having bilateral progressive Keratoconus and were already scheduled for bilateral cross-linking: Post-operatively, they were asked to apply insulin eye drops in one eye only (4 times daily) in addition to the prescribed routine post-operative treatment which was applied to both eyes. They were also asked to come for follow-up visits on post-operative day 1,2,3,5 and 7 or until complete corneal wound healing. The epithelial defect was measured in each eye and the participants were asked to report the degree of pain they felt in each eye at each visit until complete epithelial healing.
