Effectiveness and Efficiency of Home Telemonitoring and Non-invasive Ventilation in Patients With COPD and Hypercapnia (EVALECO)
The purpose of this study is to evaluate the effectiveness and clinical efficiency of combining home non-invasive ventilation (NIV) with a continuous remote telemonitoring (TM) system in patients with Chronic Obstructive Pulmonary Disease (COPD) and persistent hypercapnia. In standard practice, patients with severe COPD and high carbon dioxide levels in their blood are treated with home non-invasive ventilation to support their breathing. This study compares two different care strategies over a one-year follow-up period: 1. One group of patients will receive standard home non-invasive ventilation alone. 2. Another group will receive home non-invasive ventilation combined with an automated telemonitoring system that tracks device parameters daily (such as air leaks, device usage, and breathing rates) and sends automated alerts to the healthcare team if deviations occur. The main goal of the project is to determine if adding a telemonitoring follow-up system can prevent clinical worsenings and significantly reduce the total number of days patients spend hospitalized due to severe COPD flare-ups. Additionally, the study will analyze the impact of both strategies on patients' overall health-related quality of life and the economic costs within the healthcare system.

Expanded Studies on the SCAPIS Stockholm Reexamination Cohort
The Swedish CArdioPulmonary bioImage Study (SCAPIS) is a unique, large-scale national research initiative involving 30,000 randomly selected individuals aged 50-64, recruited between 2014 and 2018. The study is a collaborative effort among six university hospitals across Sweden. A follow-up study, SCAPIS 2, is conducted for half of the original participants. In Stockholm, 2,500 individuals will be re-examined at Danderyd University Hospital and Karolinska Institutet. SCAPIS 2 includes a core set of examinations involving blood sampling, questionnaires, and imaging. In addition to these, complementary local investigations are conducted to enable more detailed research questions. This protocol describes the additional studies conducted in the Stockholm cohort. All complementary assessments aim to identify risk factors for current and future lung, liver, and cardiovascular diseases.: EXTENDED SAMPLING: Saliva and Blood Samples with Blood Cell Isolation. EXTENDED QUESTIONNAIRES: Dyspnea, Sleep, Respiratory Infections, and Dental Health. EXTENDED IMAGING AND PHYSIOLOGICAL MEASUREMENTS Cardiac Ultrasound and Abdominal Aortic Measurements. Liver Elastography. Vascular Stiffness by cuff-based pulse wave analysis and Photoplethysmography (PPG). Valvular and Vascular Calcification by CT imaging.

RESET-COPD Cohort: Real-Time Monitoring and Early Warning in COPD Patients Receiving Home NPPV
This is a multicenter prospective cohort study designed to develop and validate a multisensor-based real-time monitoring and intelligent early warning model for acute exacerbations in patients with chronic obstructive pulmonary disease receiving home noninvasive positive pressure ventilation. Eligible participants will be adults with severe COPD and chronic hypercapnic respiratory failure who are receiving or newly initiating home noninvasive positive pressure ventilation. Participants will undergo baseline assessment and follow-up visits at 3, 6, and 12 months. Clinical data, arterial blood gas measurements, pulmonary function, exercise capacity, symptom burden, health-related quality of life, psychological status, home noninvasive ventilation parameters, adherence, device-related adverse events, healthcare utilization, acute exacerbations, hospitalizations, intensive care unit admissions, invasive mechanical ventilation, and mortality will be collected. Continuous physiological monitoring data, will be used to construct individualized stable baseline values and to identify digital warning signals. Digital warning events will be further adjudicated through symptom confirmation, treatment and healthcare utilization verification, exclusion of alternative explanations, and investigator assessment. The study aims to establish a longitudinal clinical and physiological dataset for COPD patients receiving home noninvasive ventilation and to evaluate the performance of a multisensor-based early warning model for clinically confirmed acute exacerbations of COPD.

Comparative Effects of Coherent Breathing and Regular Paced Breathing on Dyspnea and Pulmonary Function in Patients With Chronic Obstructive Pulmonary Disease
Chronic Obstructive Pulmonary Disease (COPD) is an inflammatory condition of airways characterized by recurrent episodes of coughing, respiratory muscle weakness, lung hyperinflation and air trapping which ultimately leads to dyspnea, impaired pulmonary function and reduced quality of life. Non- pharmacological approaches, particularly breathing retraining techniques, have gained recognition as complementary strategies for optimizing COPD management by improving overall functional status and exercise tolerance in the patients.

Effect of Dietary Fiber Supplementation on Systemic Inflammation and Clinical Parameters in COPD
Chronic Obstructive Pulmonary Disease (COPD) is characterized by persistent respiratory symptoms and systemic inflammation. Dietary fiber intake has been associated with beneficial effects on gut microbiota composition and systemic inflammatory markers. This study aims to evaluate the effects of dietary fiber supplementation on systemic inflammation and clinical parameters in patients with COPD through a randomized, double-blind, placebo-controlled trial.

Additional Effects of Box Breathing Technique With Short Term Pulmonary Rehabilitation on Dyspnea and Functional Capacity in Patients With Chronic Obstructive Pulmonary Disease
Chronic Obstructive Pulmonary Disease is progressive and irreversible respiratory condition characterized by persistent airflow limitation leading to dyspnea and reduced functional capacity. Pulmonary rehabilitation along with different technqiues of breathing exercises reduces dyspnea and enhance functional capacity by improving ventilatory efficiency, respiratory muscle control and overall exercise tolerance in patients with COPD.

Comparative Effects of Pilates Breathing and Pursed Lip Breathing on Chest Expension and Peak Expiratory Flow Rate in Patients With Chronic Obstructive Lung Disease
Chronic Obstructive Pulmonary Disease is a particular type of irreversible disease that may damage or obstruct airways and make it difficult to breath. This respiratiry condition may lead to shallow breathing. Breathing exercisers help to manage hyperventilation often seen in COPD patients. To handle respiratory complications patients are advised to practice breathing techniques along with pharmacological management
