Magnesium Alloy Scaffold With Controlled Degradation for Periarticular Bone Defects
This prospective, multi-center, randomized controlled clinical trial is designed to evaluate the effectiveness and safety of a biodegradable magnesium alloy scaffold with controlled degradation for the treatment of periarticular bone defects of the extremities. Adults with proximal humeral fractures, distal radius fractures, or tibial plateau fractures associated with bone defects and requiring surgical treatment will be enrolled. A total of 120 participants will be randomly assigned in a 1:1 ratio to the experimental group or the control group. Participants in the experimental group will undergo open reduction and internal fixation with implantation of a customized magnesium alloy scaffold into the bone defect. Participants in the control group will undergo conventional open reduction and internal fixation with calcium sulfate bone graft substitute used to fill the bone defect. Participants will be followed for up to 48 weeks after surgery. The study will primarily evaluate bone healing, and will also assess joint function, wound healing, degradation of the magnesium alloy scaffold, device performance, laboratory parameters, and adverse events.

Personalized Weight-Bearing With Smart Crutch Tips After Surgery for Tibial Plateau Fractures
This study will compare two approaches to rehabilitation after surgery for a fracture of the upper part of the shinbone that involves the knee joint (tibial plateau fracture). The main question is whether a personalized plan for putting weight on the injured leg, combined with feedback from smart crutch tips, helps patients return to walking without assistance sooner than usual care, without increasing complications. Participants will be assigned by chance to one of two groups: * Personalized rehabilitation: Researchers will use CT and MRI scans taken after surgery to create a computer model of the repaired bone and estimate how it responds to different loads. The treating doctor will use this information, together with the patient's clinical condition, to prescribe how much weight the patient may place on the injured leg. Smart Crutch Tips will provide feedback to help the patient follow this prescription. * Usual care: The treating doctor will prescribe weight-bearing according to usual clinical practice. Participants will also use Smart Crutch Tips, but the devices will record information without showing the patient loading data, step counts, or related alerts. Researchers will compare how long it takes participants to walk independently without crutches, a cane, or another walking aid. They will also assess bone healing, knee function, pain, swelling, fear of movement, adherence to weight-bearing instructions, and complications. Participants will attend follow-up visits and complete assessments of their recovery. Imaging specialists who do not know which treatment group participants are in will assess fracture healing and whether the repaired bone has remained in position.

Comparison of Functional Outcome Between Locking Plate Technique Versus Hybrid Ilizarov Technique in Patients With Schatzker Type V and VI Tibial Plateau Fractures
This comparative clinical study will evaluate and compare the functional outcomes of patients with Schatzker type V and VI tibial plateau fractures treated using either the locking plate technique or the hybrid Ilizarov technique. Functional outcome will primarily be assessed using the Rasmussen Knee Functional Score. Fracture healing, time to union, non-union, delayed union, and infection will also be assessed and compared between the two treatment groups.

Saphenous Nerve Block After Tibial Plateau ORIF
Fractures of the tibial plateau, while very painful, typically do not receive nerve blocks. This is because of a concern that the numbness from a nerve block would "mask" the typically painful symptoms of compartment syndrome, a condition that can lead to a permanent loss of function of the injured limb. Our study aims to evaluate the effectiveness of saphenous nerve block for pain management after surgical repair of a tibial plateau fracture. Based on cadaver studies and our clinical experience, we suspect that the saphenous nerve contributes to pain sensations of the tibial plateau and will not mask the pain from a compartment syndrome. In summary, while saphenous nerve block represents a safe option after tibial plateau open reduction internal fixation (ORIF), we aim to evaluate its effectiveness in this patient population.

Gene Expression in Lower Extremity Acute Traumatic Compartment Syndrome
The purpose of this study is to evaluate both genotypic differences and differences in local gene expression in individuals who develop acute traumatic compartment syndrome relative to control patients with at-risk lower extremity fractures who do not develop compartment syndrome.
