Age-Related Eye Disease Study (AREDS) Follow-Up

This study is a 5-year extension of the AREDS protocol, in which investigators followed the natural course of age-related macular degeneration (AMD) and cataracts. Participants in the former AREDS protocol are eligible for this study. Participants have a complete eye examination once a year and are contacted at least once a year between visits to check on their status. The eye examination includes measurement of visual acuity (vision chart test) and examination of the inside of the eye after the pupils have been dilated with eye drops. Photographs of the inside of the eye may be taken using a special camera that flashes a bright light in the eye. A blood sample may be obtained to test for cholesterol level and genes related to inflammation....

Anterior Segment Anatomy Assessed by Swept-Source OCT in Ocular Surgery Patients

This observational study aims to evaluate ocular anatomical parameters in patients undergoing anterior segment surgery using swept-source optical coherence tomography (SS-OCT). Anterior segment surgery, including procedures involving the cornea, lens, and other anterior eye structures, may be associated with changes in ocular anatomy. SS-OCT is a non-invasive imaging technique that provides detailed images of the anterior structures of the eye and allows quantitative assessment of ocular anatomical parameters. Patients undergoing anterior segment surgery at Nanfang Hospital, Southern Medical University, will be included in this study. SS-OCT will be used to assess ocular anatomical parameters, and the collected measurements will be analyzed to characterize anatomical features and explore potential differences among patients undergoing different types of anterior segment surgery. The findings of this study may improve understanding of ocular anatomical characteristics in patients undergoing anterior segment surgery and provide useful information for clinical assessment and surgical management.

A Case-Control Study Revealing Intraocular Microenvironment Characteristics in Patients With Ocular Diseases Based on Multi-Omics Analysis

This project systematically investigates the role of the intraocular microbiome in various ocular diseases, including age-related macular degeneration, diabetic retinopathy, and cataracts. By establishing a comprehensive biobank comprising aqueous humor, ocular surface, peripheral blood, and fecal samples, the study aims to: 1. Characterize the disease-specific composition, distribution, and abundance of the intraocular microbiome. 2. Identify the potential origins of intraocular microorganisms by analyzing correlations between the intraocular microecology and microbiomes at the aforementioned external sites. 3. Elucidate the regulatory mechanisms of the intraocular microbiome in disease pathogenesis utilizing multi-omics sequencing and analysis. Ultimately, this research provides novel insights into the pathological mechanisms of intraocular microecology, establishing a scientific foundation for future precision interventions and therapeutic target development.

Rebamipide Versus Diquafosol and Cyclosporine for Perioperative Dry Eye in Cataract Surgery

The goal of this clinical trial is to compare the efficacy of 2% rebamipide monotherapy versus a combination therapy of 3% diquafosol and 0.05% cyclosporine for the management of dry eye disease in patients undergoing cataract surgery. It will also evaluate the safety and ocular surface stability effects of these treatments during the perioperative period. The main questions it aims to answer are: Does rebamipide monotherapy maintain tear film breakup time (TBUT) and prevent corneal damage as effectively as the combination therapy after surgery? How do patient symptoms (OSDI scores) and inflammation markers (MMP-9) change with each treatment? Researchers will compare the rebamipide group to the diquafosol-cyclosporine combination group to evaluate their effects on perioperative dry eye management. Participants will: Use the assigned study eye drops starting 1 month before their cataract surgery and continuing for 3 months after surgery. Visit the clinic for eye examinations at baseline, the day of surgery, and at 1 week, 1 month, and 3 months postoperatively. Undergo tests including tear film breakup time measurement, corneal staining, Schirmer's test, and tear MMP-9 level assessments.

Trial Comparing miLOOP Device Assisted Phacoemulsification vs Standard Surgery for Dense Cataracts: Effects on Endothelial Cell Loss, Energy Use, and Vision Recovery

The goal of this clinical trial is to find out if using the miLOOP device during cataract surgery confers additional protection against cornea (endothelial) cells loss by reducing the amount of ultrasound energy needed. It will also look at how well patients see after surgery and how safe the procedure is. The main questions it aims to answer are: * Does the miLOOP device reduce the loss of corneal endothelial cells during and after cataract surgery? * Does it reduce the amount of ultrasound energy used to remove dense cataracts? * Are vision and safety outcomes better with the miLOOP compared to standard surgery? Researchers will compare cataract surgery using the miLOOP device to standard cataract surgery without the device. Participants with dense cataracts will: * Be randomly assigned to have cataract surgery via conventional method or the miLOOP-assisted method * Attend follow-up visits to check vision, eye cell health, and any problems after surgery

Evaluating a Dropless Postoperative Regimen After Cataract Surgery in a Vulnerable, County-hospital Population

The current postoperative cataract surgery eye drop regimen used at Zuckerberg San Francisco General Hospital (ZSFG) is a significant burden for its patient population, contributing to high rates of non-adherence and the development of postoperative complications. The investigators propose to replace this complex regimen with a single administration of intraocular antibiotic and subconjunctival steroid at the time of surgery. This pilot study will obtain the preliminary data required to eventually fully evaluate this innovation in postoperative care in a safety-net population with respect to postoperative outcomes, patient compliance, and patient and caregiver satisfaction.

Manipulating Temporal Expectations to Accelerate Recovery After Cataract Surgery

This study tested whether changing how quickly patients expect to heal after cataract surgery changes how quickly their vision recovers. Adults having cataract surgery in both eyes were randomly assigned to one of two groups. Both groups watched a short video of their surgeon before each surgery and completed short daily surveys afterwards. In one group, the video gave somewhat faster recovery timeframes and the surveys asked patients to notice day-to-day changes in their vision. In the other group, the video gave standard recovery timeframes and the surveys asked about activities and mood. Visual acuity was measured at routine clinic visits 1 day, 1 to 2 weeks, and 3 months after surgery.

Dexamethasone Versus Metoclopramide for Prophylaxis of Nausea and Vomiting After Cataract Surgery

Postoperative nausea and vomiting (PONV) is a common consequence of ambulatory surgery, affecting up to 80% of high-risk patients and 20-30% overall. Despite being widespread, a prior survey found that patients are more averse to PONV than pain and other significant postoperative problems. Furthermore, even mild cases of PONV can have serious repercussions, such as postanesthesia care unit (PACU) workflow disruption, delayed discharges, higher medical costs, and lower patient satisfaction.PONV following intraocular surgery can be unpleasant and may lead to serious complications including suprachoroidal hemorrhage (SCH). SCH is a potentially serious condition that occurs when the posterior ciliary arteries or vortex veins burst, resulting in blood in the suprachoroidal region. The hypothesis is that dexamethasone will lower the incidence of PONV after cataract surgery when compared with metoclopramide.

Postoperative Outcome Verification of Discrepant Toric IOL Calculator Recommendations After Cataract Surgery

The goal of this observational study is to learn which toric lens calculator best predicts the astigmatism remaining after cataract surgery in adults who received a TECNIS DIU toric intraocular lens (IOL) at Klinik Hietzing, Vienna, between January 2023 and March 2026. Toric lenses correct astigmatism (an uneven curvature of the cornea) during cataract surgery. The strength of the lens and its orientation in the eye are chosen with online calculators. In an earlier study of the same patients, two widely used calculators - the Johnson \& Johnson toric calculator and the Barrett calculator of the European Society of Cataract and Refractive Surgeons (ESCRS) - recommended different lenses in about 6 out of 10 eyes. It is not known which recommendation leads to the better result. The main questions it aims to answer are: 1. In eyes in which the two calculators disagreed, which calculator predicted the astigmatism after surgery more accurately? 2. How accurate are four further calculator options, and did the toric lenses stay in their planned position? Researchers will compare the astigmatism that each calculator predicted for the lens that was actually implanted with the astigmatism measured after surgery. Participants will: 1. Attend one examination visit of about 60 to 90 minutes, at least 3 months after their cataract surgery 2. Have standard eye examinations: a vision test, a test for glasses (refraction), and several painless optical scans that do not touch the eye 3. Have the eye examined at the slit lamp; drops to widen the pupil are given only at the end of the visit, if needed, to check the position of the lens No treatment, medication or surgery is given as part of this study.

Study on Eye-Brain Cross-Organ Mapping and Systemic Disease Association Based on Multimodal Big Data

This project integrates multimodal eye data (CFP, OCT, OCTA) from 50,000 cases, brain MR data from 150,000 cases, and ICD diagnoses, medical orders, and test results from an eye-brain paired cohort of 8,000 cases to construct an eye-brain cross-modal mapping model and an eye-brain-systemic disease association model. It aims to clarify the quantitative associations between multimodal ocular features and brain structural and vascular characteristics as well as systemic disease ICD diagnoses, thereby uncovering the cross-organ and cross-modal linkage mechanisms between the eye and brain. Ultimately, it seeks to achieve mapping and prediction of brain imaging features based on ocular data, enable ocular-based diagnosis and prediction of various diseases, and contribute significantly to early disease screening, risk stratification, and optimization of clinical diagnosis and treatment.

Imaging and Spectral Analysis of the Crystalline Lens Using the "HYPERLAF" Slit Lamp

Cataract is the leading cause of reversible blindness and the first surgery worldwide. It corresponds to the crystalline lens opacification. Nevertheless, its grading remains mostly subjective and the Lens Opacities Classification System dating back 1993, is not used in daily practice and has not been revisited since. The HYPERLAF device developed by the start-up HYPEREYE is a modified slit lamp which combines imaging and spectral analysis, providing two quantitative objective scores: OPAK (opacity) and COLOR (spectral analysis). It is a non-CE marked prototype with non-invasive imaging methods.
