Trigger Timing in Ovarian Stimulations
The gonadotrophin-releasing hormone (GnRH) antagonist protocol is widely used in in vitro fertilisation (IVF) for its flexibility and ability to reduce the risk of ovarian hyperstimulation syndrome (OHSS). Conventionally, the trigger criterion is set at ≥3 follicles ≥17 mm. Whether this criterion is premature and whether a 1-2 day delay could improve the number of good-quality embryos and cumulative live birth rate, requires prospective evaluation. While delaying the trigger could theoretically allow for more complete follicular maturation and improve oocyte yield, existing evidence is contradictory; some studies report increased oocyte numbers without a corresponding improvement in live birth rates, potentially due to adverse endometrial changes. Therefore, this randomised controlled trial (RCT) aims to determine whether a one- to two-day delay in triggering ovulation, compared to the conventional criterion, increases the number of good-quality embryos and improves cumulative live birth rates in women undergoing IVF or intracytoplasmic sperm injection (ICSI).

Comparing IVF/ICSI Cycle Outcomes of Post Ovulation Ovarian Stimulation Protocol With Elonva Without GnRH Antagonist to Follicular Phase Day 2 Stimulation Fixed Day 5 Antagonist Protocol
There are several approaches to the timing of antagonist administration in IVF/ICSI cycles. The primary goal is indeed to prevent premature ovulation, which could jeopardize the success of the cycle There is an emerging concept that suggests that the progesterone produced by the corpus luteum (formed after ovulation) might be sufficient to prevent further ovulation, obviating the need for antagonist or exogenous progesterone administration. This hypothesis relies on the natural regulatory mechanisms of the menstrual cycle to maintain a progesterone-dominated environment post-ovulation. We hypothesized that this approach would minimize treatment costs /burden without having an impact on the outcome.

The ROAR Study: Reactive Oxygen Species and Reproduction
This is a prospective cohort study characterizing follicular fluid and serum levels of oxidative stress in women with various infertility diagnoses undergoing in vitro fertilization, and assessing the relationship of oxidative stress to oocyte competence as determined by embryo development outcomes. Furthermore, we will measure oxidative stress in sperm and correlate the overall oxidative stress within the couple with IVF and pregnancy outcomes of the resultant embryos.
