Union-LC-MPE Cohort:A Multicenter Cohort Study on Diagnosis and Prognosis of Malignant Pleural Effusion in Lung Cance
Lung cancer is the malignant tumor with the highest incidence and mortality rates in China. Malignant pleural effusion (MPE) is one of its most common complications, affecting approximately 15%-50% of patients with non-small cell lung cancer at initial diagnosis or during disease progression and indicating advanced-stage disease with a median survival of only 3-12 months. This study aims to establish the first large-scale, multicenter LC-MPE disease-specific cohort, integrating clinical, imaging, pleural fluid laboratory, pathological and genomic multimodal data to develop an AI-assisted model for the differential diagnosis of benign versus malignant MPE and a prognostic prediction model for LC-MPE patients. The study comprises a retrospective phase (3,000 patients diagnosed between January 2022 and September 2026) and a prospective phase (200 patients diagnosed between October 2026 and October 2028). The primary endpoint is overall survival (OS); secondary endpoints include pleural effusion progression-free survival (PE-PFS), duration of pleural effusion control, and 60-day and 30-day MPE recurrence rates. This observational study does not interfere with routine clinical care and strictly protects the personal information of participants.

Impact of Lung Cancer Screening With Chest Scans Combined With Coaching on Smoking Cessation in Heavy Smokers
Lung cancer is common and serious, but when detected early, the chances of recovery are significantly better. Tobacco use is the leading cause. To help people break free from smoking, the Fribourg Cantonal Hospital (HFR), the Center for Tobacco Prevention (CIPRET), and the Fribourg Cancer League are joining forces in the STOP-LungCancer study (Smoking Termination Outcomes with Pulmonary Low Dose Computed Tomography for Lung Cancer Screening). What is the goal of this study? The investigators want to determine whether seeing and understanding images of the participants' own lungs helps participants quit smoking for good. Smoking cessation will be verified at 6 months using a breath test (measuring carbon monoxide), followed by a 2-year follow-up. How does participation work? The study is open to 116 residents of the canton of Fribourg aged 50 to 79 who are heavy smokers and motivated to quit. The program includes: 1. Personalized support: 4 smoking cessation coaching sessions organized by CIPRET. 2. Lung computed tomography (CT) scans: 3 exams spaced one year apart. This scan involves no needles or injections. The radiation dose is very low, about 0.6 millisieverts (mSv) per scan, and the associated risk is considered very low. 3. A review of the images with a doctor: During dedicated consultations, a thoracic radiologist reviews the participant's own computed tomography (CT) scans with the participant to explain the condition of the lungs and heart. What are the benefits? Participants receive both tailored support to quit smoking and close monitoring to detect any potential issues at a very early stage.

Functional Lung Avoidance Planning Guided by Lung Perfusion PET/CT Versus Anatomical Planning for Lung Stereotactic Body Radiotherapy
Stereotactic body radiotherapy (SBRT) has an increasing role in the treatment of both primary and secondary lung tumors. However, lung SBRT remains associated with significant radiation induced lung injury (RILI). Indeed, the reported incidence of symptomatic radiation induced lung injury (grade≥2) in the published literature is up to 20%. A current challenge of lung SBRT is therefore to better preserve lung function and to reduce pulmonary toxicity. During standard lung SBRT planning, dose constraints are defined on the anatomical lung volume. This planning considers the lung as functionally uniform and does not take into account the variability of regional lung function distribution. Functional lung avoidance is an emerging concept in lung radiotherapy (RT). The technique aims at personalizing RT treatment planning to individuals' lung functional distribution, by sparing functional pulmonary areas while prioritizing delivery of high doses to non-functional regions. 68Ga-MAA lung perfusion PET/CT is a novel imaging modality for regional lung function assessment. As compared with conventional lung scintigraphy, lung perfusion PET/CT is inherently a vastly superior technology for image acquisition (higher sensitivity and spatial resolution, greater access to respiratory gated acquisition). A more accurate lung functional mapping improves the possibility of functional lung avoidance planning for SBRT. The hypothesis is that functional lung avoidance planning guided by 68Ga-MAA perfusion PET/CT, while delivering an optimal dose to the tumor, will reduce the frequency of RILI in patients treated with lung SBRT.

Effect of Infrared-Assisted Intravenous Catheterization on Patient Comfort in Lung Cancer Patients
This study evaluated the effect of near-infrared (NIR) vein visualization-assisted peripheral intravenous catheterization on procedure-specific patient comfort and procedural pain in adults with lung cancer receiving outpatient chemotherapy. Patients receiving chemotherapy often develop progressive venous damage, which can make peripheral intravenous catheterization difficult. Failed attempts may increase pain and discomfort. The study used an explanatory sequential mixed-methods design (QUAN→qual). In the quantitative phase, 160 patients were randomized 1:1 to NIR-assisted catheterization or to standard catheterization based on visual inspection and palpation. In the qualitative phase, semi-structured individual interviews were conducted with 11 patients from the intervention group to explore their experiences with the NIR device. The study was conducted at the Outpatient Chemotherapy Unit of Ataturk University Research Hospital, Erzurum, Turkey, as part of a doctoral thesis in Fundamentals of Nursing.

Symptoms, Physical Function, and Recovery in Patients Receiving Lung Cancer Treatment Before Surgery
This study will examine how symptoms and physical abilities change during lung cancer treatment given before planned surgery, and how these changes relate to recovery after surgery. Treatment given before surgery is called neoadjuvant therapy. The main questions are: * How do symptoms and the ability to perform everyday activities change during treatment and before surgery? * Are symptoms and reduced physical function before surgery associated with lung complications or slower recovery afterward? * Can patient-reported information add useful information to routine preoperative assessment? Participants will complete questionnaires before starting neoadjuvant therapy, during treatment, before surgery, and during recovery after surgery. The questionnaires will assess symptoms such as fatigue, breathlessness, and pain, along with physical abilities and well-being. Researchers will also collect information from medical records about cancer treatment, lung function, anesthesia, surgery, pain management, and postoperative complications. Changes to the planned treatment pathway, including delays or cancellation of surgery, will be recorded. This is an observational study. Cancer treatment, surgery, and anesthesia will be determined by the treating clinical teams, not assigned by the research team. The findings may help identify patients who need additional assessment and support before and after lung surgery.

MIND-WELL Intervention
Lung cancer patients often go through more than one type of treatment, such as chemotherapy, radiation, and immunotherapy. These treatments can lead to several transition periods, which may increase stress and leave some needs unmet for patients and their family caregivers. The purpose of this study is to find out whether a program that includes mindfulness (stress reduction) and education is helpful, acceptable, and practical for lung cancer patients and their family caregivers. We also want to learn if this program helps them feel better prepared during these transitions.

Accurate Planning and Guidance of Bronchoscopy for Lung Cancer Management
The main objective of this project is to demonstrate the potential of three new technical innovations for improving state-of-the-art bronchoscopy guidance systems. Through this study, we plan to generate preliminary data that demonstrates the potential of three new innovations for enhancing existing bronchoscopy systems in the live clinical setting. The goal is to demonstrate the potential of the new innovations: 1. to demonstrate the potential of a new methodology to reduce errors from CT-to-body divergence in the live clinical setting, 2. to demonstrate the potential of the new methodologies to enable live rerouting of a guided bronchoscopy, and 3. to perform automatic real-time mucosal lesion detection during a standard airway examination. These innovations could potentially improve current image-guided bronchoscopy systems and be used for future robotic bronchoscopy.

Iriscope for the Diagnosis of Peripheral Pulmonary Lesions: a Multicentre Randomised Controlled Trial
A pulmonary nodule is a small mass detected in your lungs during a scan. When a nodule is suspicious, we need to determine its nature (benign or malignant). To do this, a bronchoscopy is performed. Bronchoscopy is a common examination. It involves inserting a thin, flexible tube (the bronchoscope) into the airways to take tissue samples (biopsies) from the nodule. This examination is routinely performed in the hospitals participating in this study. Currently, bronchoscopy is guided by fluoroscopy, a type of real-time X-ray imaging. However, sometimes we are unable to directly see the nodule, which can reduce the chances of obtaining an accurate diagnosis. The Iriscope is a small device, 1.3 mm in diameter, that slides into the bronchoscope. It allows direct visualization of the inside of the small bronchi and the nodule. Its purpose is to help us navigate more precisely and thus obtain a better diagnosis. The Iriscope carries the CE mark. This mark guarantees that it meets European safety standards for medical devices. The purpose of this study is to compare the rate of correct diagnosis between bronchoscopy guided by fluoroscopy alone and bronchoscopy guided by a combination of fluoroscopy and the Iriscope.

Registry of Minimally Invasive Cancer Treatment Using Spectral Angio-CT Image Guidance
This study collects data from patients undergoing minimally invasive, image-guided cancer treatments at participating hospitals in the Netherlands and France. All treatments are performed as standard clinical care and include procedures such as tumor ablation and catheter-based treatments of cancer in the liver, kidney, lung, and bone. During these procedures, a spectral angio-CT system is used for image guidance. This advanced CT scanner automatically captures additional imaging information beyond what is used in routine clinical decision-making. The goal of this study is to investigate how this additional imaging information is associated with treatment outcomes, and how it can be used in the future to improve treatment planning, procedural guidance, and prediction of treatment results. No experimental interventions are performed. Participation does not alter the treatment or follow-up patients receive. Data are collected from medical records and imaging systems and stored securely for analysis.

Effect of Mechanical Power-based Lung-protective Ventilation During One-lung Ventilation
The goal of this clinical trial is to determine whether a mechanical power-based lung-protective ventilation strategy during one-lung ventilation reduces the incidence of postoperative pulmonary complications in adult patients undergoing lung resection surgery. Mechanical power during pressure-regulated volume control mode is calculated using the formula below. Mechanical power (J/min) = 0.098 x respiratory rate x tidal volume x (inspiratory airway pressure above positive end-expiratory pressure + positive end-expiratory pressure) The main questions it aims to answer are: * Does a mechanical power-based lung-protective ventilation strategy significantly lower the incidence of major postoperative pulmonary complications compared to conventional ventilation? * How does this strategy impact intraoperative hemodynamic and vital profiles (e.g., blood pressure, cardiac output, vasopressor requirements, and blood oxygen saturation) during one-lung ventilation? Participants will: * Be randomized to receive either a mechanical power-based lung-protective ventilation strategy or a conventional lung-protective ventilation strategy exclusively during one-lung ventilation. * Be closely monitored for the occurrence of major postoperative pulmonary complications (atelectasis, pneumonia, acute respiratory distress syndrome, and pulmonary aspiration) and adverse events for 1 month postoperatively.

Combination Versus Alone Liposomal Bupivacaine Blocks in Minimally Invasive Thoracic Surgery-1
The goal of this clinical trial is to assess how the location of local anesthetic medication affects pain control capabilities in adult patients undergoing thoracic(chest)-only minimally invasive surgery. The main questions it aims to answer are: * Is pain decreased when local analgesia is provided as a combination of serratus anterior plane block and intercostal nerve block when compared to either block administered individually? * How does location of local analgesia administration impact length-of-stay, cost-of-care, and quality-of-life? Researchers will compare three treatment options including long-acting local anesthetic (Exparel®) administered as (1) intercostal nerve block, (2) serratus anterior plane block, or (3) combination of intercostal nerve block and serratus anterior plane block to see if pain and opioid use are reduced dependent upon treatment group. All participants will receive a local anesthetic nerve block. Participants will be asked to describe their pain after surgery and to answer various questionnaires before and after surgery.
