A Study of MC001 in Patients With Advanced or Metastatic Pancreatic Ductal Adenocarcinoma (PDAC) and Other Folate Receptor Alpha (FRα)-Expressing Solid Tumors
The purpose of this First-in-Human, Phase 1 study is to evaluate the safety, tolerability, pharmacokinetics (PK), pharmacodynamics (PD), immunogenicity, and preliminary antitumor activity of MC001 in adults with advanced or metastatic pancreatic ductal adenocarcinoma (PDAC) and other solid tumors that have relapsed or are refractory to standard therapies. MC001 is an investigational dimeric antibody-drug conjugate (ADC) that targets folate receptor alpha (FRα) and delivers a microtubule-disrupting cytotoxic payload, monomethyl auristatin E (MMAE). The study will be conducted in two sequential parts: Part A (Dose Escalation): Evaluates ascending intravenous (IV) dose levels of MC001 administered once every 3 weeks (Q3W) using a standard 3+3 design in patients with advanced solid tumors to identify dose-limiting toxicities (DLTs), the maximum tolerated dose (MTD), and the recommended dose for expansion (RDE). Part B (Dose Expansion): Evaluates MC001 at the selected RDE in patients with advanced or metastatic PDAC whose tumors express FRα on at least 30% of tumor cells to further characterize safety and preliminary efficacy.

NCT07030283
Intraperitoneal Paclitaxel With NALIRIFOX for Pancreatic Ductal Adenocarcinoma With Peritoneal Carcinomatosis
This goal of this clinical trial is to learn whether the drug combination of intraperitoneal paclitaxel (chemotherapy given directly into the abdominal cavity) and intravenous NALIRIFOX (chemotherapy given into a vein, including fluorouracil, leucovorin, oxaliplatin, and liposomal irinotecan) is safe and works in adults with pancreatic cancer that has spread to the peritoneum. The main questions it aims to answer are: * Are people with pancreatic cancer able to tolerate the combination drug regimen? * How well does the combination drug regimen work to treat pancreatic cancer? Participants will: * Obtain a port that goes into the abdomen to deliver intraperitoneal paclitaxel (called an intraperitoneal catheter) * Receive treatment with intravenous NALIRIFOX once every 2 weeks and intraperitoneal paclitaxel on days 1 and 8 of each 14-day cycle * Visit the clinic with each treatment for checkups and laboratory testing * Have imaging scans and blood lab testing to determine response to treatment * Have abdominal fluid lab testing that may help determine if the cancer is responding to treatment * Fill out questionnaires to see how the treatment affects how participants feel and function * Continue follow up after treatment ends to track survival Some participants may be able to have surgery later if the cancer responds well. This is called conversion surgery. To be eligible for surgery, the cancer must have shrunk or stayed the same, peritoneal fluid (from the abdomen) must no longer show cancer cells, and a tumor marker called CA 19-9 must decrease or return to normal. The decision to do surgery will depend on the treating surgeon. By testing this new treatment strategy, researchers hope to find a safer and more effective way to treat people with pancreatic cancer that has spread to the abdomen. If successful, this approach may lead to longer survival, better quality of life, and more people becoming eligible for surgery.

AI-Assisted Differentiation of Autoimmune Pancreatitis and Pancreatic Cancer Using EUS: A Multicenter Ambispective Study
Focal autoimmune pancreatitis (f-AIP) and pancreatic ductal adenocarcinoma (PDAC) exhibit substantial overlap in clinical manifestations and imaging features, posing a major diagnostic challenge in pancreatic diseases. Given the marked differences in treatment strategies and prognosis, accurate differentiation is essential to avoid unnecessary surgery and ensure timely initiation of anticancer treatment. This study aims to collect endoscopic ultrasound (EUS) images from patients with confirmed f-AIP or PDAC across multiple centers, using a previously established multicenter database. Data collected before January 2026 will be used to develop an artificial intelligence (AI) model based on EUS radiomics for differential diagnosis. Subsequently, prospectively collected data from participating centers from 2026 onward will be used for model testing and validation. The diagnostic performance of the model will be evaluated and compared with that of endoscopists with different levels of experience. Furthermore, this study will investigate the clinical value of interpretable radiomic features, with particular emphasis on challenging diagnostic scenarios, including IgG4-negative AIP, AIP with elevated carbohydrate antigen 19-9 (CA19-9) levels, and PDAC with imaging features resembling those of AIP. The ultimate goal is to provide a reliable tool for accurate, noninvasive preoperative differential diagnosis while promoting the standardization of multicenter AIP databases and the sharing and utilization of imaging data resources.

Reach and Engagement of GENetic Education, Risk Assessment, and TEsting Feasibility Trial
This study will test a remote approach to help people who may be at higher risk for pancreatic cancer learn about and decide whether to get genetic testing and a blood test that looks for signs of several cancers (MCED testing). The study is focused on Black, Latino/a, and Medicaid-enrolled individuals who may have had less access to cancer genetics and early detection services. About 80 participants, who reside in Massachusetts, will be assigned by chance to receive either online education alone or online education plus a phone call with a trained educator from the Pancreatic Cancer Action Network (PanCAN). After the education, participants can choose whether or not to have genetic testing and/or MCED testing. The main goal is to learn whether this approach is practical to carry out, acceptable to participants, and delivered as planned. Participants will complete 3 surveys during study participation.

European Appleby Registry for Pancreatic Cancer
The EURO-APPLEBY Registry is a prospective, multicenter European observational registry designed to characterize contemporary surgical practice and evaluate perioperative and long-term oncological outcomes in patients undergoing distal pancreatectomy with celiac axis resection (DP-CAR), also known as the modified Appleby procedure, for pancreatic ductal adenocarcinoma of the body and/or tail. Consecutive eligible patients undergoing elective DP-CAR with curative intent will be prospectively enrolled at participating European hepatopancreatobiliary centers over a 24-month recruitment period. Clinical, treatment-related, imaging, operative, pathological, postoperative, and follow-up data will be collected using standardized definitions and a harmonized electronic case report form. The primary objectives are to characterize postoperative morbidity and mortality, overall survival, and disease-free survival following DP-CAR. Secondary objectives include identifying factors associated with postoperative ischemic complications and oncological outcomes, evaluating the impact of neoadjuvant therapy and surgical approaches, and assessing variability in clinical practice and outcomes across participating centers. This observational registry will not modify routine clinical care. The findings are expected to support benchmarking, improve patient selection and perioperative management, and inform the development of evidence-based European recommendations for DP-CAR.

Breath Research Narrow Validation for Gastrointestinal Cancer Detection
The investigators of this study are developing a simple breath test to help detect gastrointestinal (gut) cancers earlier, including cancers of the oesophagus (food pipe), stomach, pancreas, liver and bowel. These cancers often cause non-specific symptoms that are similar to benign conditions, making early diagnosis difficult. Delays between the onset of symptoms and referral for a diagnostic test such as an endoscopy or a scan, can allow the cancer to progress. The breath test detects small molecules called volatile organic compounds (VOCs) that are released in exhaled breath. Some of these VOCs are strongly associated with these cancers and may help identify high-risk patients who require urgent investigation. In practice, patients who come to their GP with concerning symptoms will be offered the breath test. If the test is positive, patients can be referred promptly for a diagnostic test, while those with a negative result can be reassured and offered re-testing if symptoms persist. Earlier diagnosis could improve access to curative treatment while reducing unnecessary invasive investigations. Previous studies have identified a panel of VOCs that appear to distinguish patients with gastrointestinal cancers from those without cancer. This study aims to confirm these findings in a new group of participants to determine whether the same biomarkers can be reliably identified. Participants will provide a breath sample, usually before a hospital procedure they are already scheduled to undergo, and complete a short questionnaire about their medical history and medications. Some participants will also be asked to drink a nutritional supplement before providing a second breath sample. The results will help determine whether the breath test is reliable enough for further clinical evaluation

Clinical Study of Radiotherapy Combined With Adebrelimab and NALIRIFOX for Conversion Therapy in Locally Advanced Pancreatic Cancer
This study intends to explore conversion therapy for locally advanced pancreatic cancer (LAPC). The intervention starts with modified stereotactic body radiation therapy (3-day regimen), followed by systemic therapy within 1-2 weeks after irradiation. Subsequently, the investigator will evaluate the resectability of the lesion. For subjects converted to resectable status, surgical resection will be performed, followed by postoperative adjuvant therapy. For subjects who remain unresectable but maintain stable disease, a second cycle of conversion therapy will be continued. If the lesion is converted to resectable, surgical resection will be performed followed by adjuvant therapy. If disease progression occurs, subsequent treatment will be carried out in accordance with the first-line treatment for advanced pancreatic cancer. For subjects who experience disease progression or distant metastasis, subsequent treatment will be administered by adjusting the chemotherapy regimen in accordance with the first-line standard regimen for advanced pancreatic cancer.
