Multimodal Assessment of NMES Effects on Respiratory and Peripheral Muscles in COPD

The goal of this clinical trial is to learn if targeted neuromuscular electrical stimulation (NMES) can treat respiratory and peripheral muscle weakness in patients with chronic obstructive pulmonary disease (COPD). The main questions it aims to answer are: * Does targeted NMES improve diaphragm thickening fraction and excursion as measured by diaphragm ultrasound? * What are the comparative effects of targeted NMES on plasma biomarkers, specifically Interleukin-6 (IL-6) and Insulin-like Growth Factor-1 (IGF-1)? Researchers will compare three groups: the first intervention group receiving intercostal NMES combined with Pursed-Lip Breathing (PLB), the second intervention group receiving quadriceps NMES combined with PLB, and a control group receiving sham upper trapezius NMES combined with PLB, to see if targeted NMES provides superior improvements in respiratory and peripheral muscle function. Participants will: * Undergo baseline screening, including a diaphragm ultrasound and a blood draw for plasma biomarkers (IL-6 and IGF-1) * Be randomly assigned to one of the three groups to receive either intercostal NMES, quadriceps NMES, or sham upper trapezius NMES, all combined with Pursed-Lip Breathing exercises * Complete a post-intervention assessment involving repeat ultrasound imaging and biomarker analysis to evaluate muscle and systemic changes

Lungs on Fire: Wildfire Smoke, Incident Diseases, Susceptible Populations, and Community Values in Canada

Wildfire smoke (WFS) is the leading climate-related risk in Canada and the main source of harmful air pollution. While short-term breathing problems caused by smoke are well known, there is limited knowledge on how repeated exposure contributes to long-term lung disease. This study is a controlled human exposure to varying concentrations of WFS in a safe setting. By comparing the effects of different concentrations, this research will improve understanding of health impacts, identify who may be most vulnerable to exposures, and explore biological changes that could lead to chronic illness.

Collection of Airway, Blood and/or Urine Specimens From Subjects for Research Studies

The purpose of this study is to obtain biologic materials from the blood, airways and/or urine of normal individuals and individuals with lung disease. The normal are used to establish a set of normal ranges for various parameters. These provide control information when compared to individuals with various pulmonary diseases, and will help in understanding of the etiology and pathogenesis of various lung diseases. The underlying hypothesis is that the pathologic morphological changes in the airway epithelium must be preceded by changes in the gene expression pattern of the airway epithelium and potentially in macrophages.

A Study on COPD Phenotypes, Endotypes and Treatable Traits

Chronic Obstructive Pulmonary Disease (COPD) is a heterogeneous and multisystemic disease with progressive increasing morbidity and mortality. COPD is now widely accepted as a heterogeneous condition with multiple phenotypes and endotypes. The recognition of COPD phenotypes and endotypes has significantly impacted the management of the disease. Furthermore, treatable traits (TTs) are recognizable phenotypic or endotypic characteristics that can be assessed and successfully targeted by therapy to improve a clinical outcome in a patient with airway disease. The healthcare system in Hong Kong is different from that of other places. Previous studies in Hong Kong only looked at specific endotypes; an example is the eosinophils cut-off to predict COPD exacerbations and the treatment effect of applying interventions to certain phenotypes, e.g. approaching the exacerbation phenotype with a comprehensive care programme. There is not much information on the overall phenotypes, endotypes, and TTs of COPD patients in Hong Kong. The characteristics of the patients would help to further assess whether the multi-TT approach can improve their outcomes. This study aims to assess 1. the phenotypes, endotypes and treatable traits of COPD patients over time 2. Outcome of patients according to their phenotypes, endotypes and treatable traits over a period of 3 years. 4\) Clustering analyses will also be employed to assess groups of patients with similar characteristics and to assess the change in their outcome over time.

PBMT Associated With a Pulmonary Rehabilitation Program on the Functional Capacity of Patients With COPD

This study aims to evaluate the effectiveness of photobiomodulation therapy (PBMT) as an adjunct to the pulmonary rehabilitation program on the functional capacity of patients with COPD. This is a randomized, placebo-controlled, double-blind clinical trial conducted with 20 patients from the outpatient clinic of the Integrated Cardiopulmonary and Metabolic Rehabilitation Service of Santa Casa de Porto Alegre. Patients will be allocated to the active PBMT group, receiving photobiomodulation treatment prior to the exercise protocol of the pulmonary rehabilitation program, in the respiratory and peripheral muscles, or the placebo group, which will receive the therapy application at the same points but without the therapeutic dose, prior to rehabilitation. Sixteen PBMT applications will be performed over eight weeks, with a frequency of twice a week. The exercises performed in the institution's pulmonary rehabilitation program by the patients will not be interfered with by the researchers. Evaluations will be carried out in three stages: immediately after recruitment, after 8 interventions, and after 16 interventions.
